• last name typically goes on the back of the jacket and first name on the front.
• When completing form below make sure to enter names as they coorespond to the jacket combo style choosen.
• If you have a patch contact us when you receive your patch and we will sew it on.
• Contact us after you purchase your jacket and if you wish to schedule a fitting
